Continuity when managers change
A passport keeps agreed support available when reporting lines or teams change.
A workplace adjustment passport is a record of an employee's agreed adjustments and working preferences that travels with them through the organisation.
When a manager changes, the employee can share the current passport where appropriate. The record helps the new manager understand what support has been agreed.
Organisations use several terms for related records, including workplace adjustment passport, reasonable adjustment passport, disability passport, adjustments passport, workplace adjustments plan and workplace adjustments agreement. These terms are not legally defined or interchangeable in every context, and the Equality Act 2010 does not require a particular format.

Employees can share an existing record rather than restarting the full explanation with each new manager.
A shared format reduces reliance on informal arrangements and individual memory.
A static document records an outcome, but it does not show how a request was discussed or decided, track whether actions happened, flag a review date, or provide organisation-wide reporting. Documents can also become outdated or difficult to find.
A digital passport is easier to access, share securely and update. It becomes more useful when connected to a workflow that manages requests, decisions, implementation and review.
A passport records agreed support. Active adjustment management structures the process behind that record: identifying a barrier, requesting support, discussing options, recording a decision, implementing actions and reviewing what works.
Subject to role-based access and organisation configuration, an authorised new manager can use the relevant history of what was requested, discussed and agreed, including recorded rationale, to understand how the current support was reached.
Needs, roles and working environments change. TryMosaic supports scheduled reviews so the record can be revisited and updated instead of remaining a snapshot from one point in time.
Consistency does not mean identical outcomes. It means each request follows a clear process for discussion, decision and recording, while the individual facts are considered.
